Essam Abdelhakim Grey Cases in Internal Medicine

Modern medicine is not black and white. It is grey. Every junior doctor and GP quickly discovers that real patients rarely present like textbooks or exam questions. Symptoms are vague, tests are normal, imaging is inconclusive, and yet something does not feel right. These are the grey cases -the everyday clinical scenarios where uncertainty is high, risk is hidden, and poor decisions can have serious consequences. Grey Medicine is a practical, real-world guide designed for junior internal medicine doctors, foundation doctors, GP trainees, and primary care clinicians who face diagnostic uncertainty on busy wards, during night shifts, and in general practice. This book focuses on the situations that guidelines struggle to cover: chest pain with normal ECG and troponin, shortness of breath with clear lungs, fever with no source, confusion in older adults, unexplained abdominal pain, and other high-risk presentations with reassuring initial tests. Rather than teaching rare conditions or exam-style answers, this book teaches clinical judgement , safe decision-making , and how to think when you are unsure . Each chapter breaks down common grey presentations, highlights red flags, exposes cognitive traps, and provides a structured approach to assessment, escalation, documentation, and safety-netting. If you are a junior doctor, GP, or primary care clinician who wants to practise safely, avoid missed diagnoses, and gain confidence managing real patients-not ideal ones-this book is for you. Grey medicine is where good doctors are made.
